Invitation to bid supply and delivery of disposable pillow under framework agreement early procurement activity ib no. 25-fa-101 1 the eastern visayas medical center, through the nep 2026 intends to apply the sum of php6,000,000.00 being the approved budget for the contract abc to payments under the contract for supply and delivery of disposable pillow under framework agreement early procurement activity. this is an early procurement activity. issuance of notice of award/nefa upon approval of gaa 2026. bids received in excess of the abc shall be automatically rejected at bid opening. 2 the eastern visayas medical center, now invites bids for the above procurement project. delivery for disposable pillow shall be within 15 days upon receipt of approved call off. bidders should have completed, at least five 5 years from the date of submission and receipt of bids, contracts similar to the project. the description of an eligible bidder is contained in the bidding documents, particularly, in section ii. instructions to bidders. 3 bidding will be conducted through competitive bidding procedures using a non- discretionary pass/fail criterion as specified in the irr of ra no. 12009. bidding is restricted to filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent 60% interest or outstanding capital stock belonging to citizens of the philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to filipino citizens, pursuant to ra no. 5183. 4 interested bidders may obtain further information from eastern visayas medical center and inspect the bidding documents at the address given below from 8:00 am to 5:00 pm. 5 a complete set of bidding documents may be acquired by interested bidders on december 04, 2025 from the address given below upon payment of the applicable fee for the bidding documents, pursuant to the latest guidelines issued by the gppb, in the amount of ten thousand pesosphp10,000.00. it may also be downloaded free of charge from the website of the philippine government electronic procurement system philgeps and the website of the procuring entity, provided that bidders shall pay the applicable fee for the bidding documents not later than the submission of their bids. procedure on how to purchase the bidding documents. 1. secure order of payment at the procurement section/bac secretariat. 2. pay the bidding documents fee at the evmc cash operation section. 3. submit a copy of the official receipt to the procurement section/bac secretariat 4. a complete set of bidding documents shall be provided or emailed to the bidder upon receipt of the official receipt. if payment of bidding document fee is paid thru direct bank deposit/bank transfer: 1. request order of payment and evmc land bank account details at the procurement section/bac secretariat at evrmc.bacsec@gmail.com before paying the bid documents fee. 2. deposit/transfer the amount indicated in the order of payment to evmc land bank account. bidder to write in the deposit slip upper middle portion the name of the company and name of authorized representative. 3. send a scanned copy of the teller validated deposit slip to evrmc.bacsec@gmail.com on the day the bidder deposited the bid documents fee. note: depositor should be the name of the company with the name of authorized representative indicated. 4. a complete set of bidding documents shall be emailed upon receipt of the scanned deposit slip. 6 the eastern visayas medical center will hold a pre-bid conference on december 12, 2025 at 3:00 pm at bac conference room, second floor, admin. building, eastern visayas medical center, tacloban city and/or through video conferencing which shall be open to prospective bidders. 7 bids must be duly received by the bids and awards committee bac secretariat through manual submission at the office address indicated below, on or before january 06, 2026, 2:00 pm. late bids shall not be accepted. 8 all bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in itb clause 16.1. 9 bid opening shall be on january 06, 2026, at 2:00 pm at bac conference room, second floor, new admin. building, eastern visayas medical center, tacloban city. bids will be opened in the presence of the bidders representatives who choose to attend the activity 10 the schedule of bidding activities are as follows: activities schedule advertisement / posting of invitation to bid december 04, 2025 issuance and availability of bid documents december 04, 2025, onwards, 8:00 am to 5:00 pm pre-bid conference december 12, 2025 at 3:00 pm issuance of supplemental bid bulletin if any december 29, 2025 opening of bids january 06, 2026 at 2:00 pm 6 the eastern visayas medical center reserves the right to
