Procurement of one 1 lot supply and delivery of various construction materials for the dotr-co. please see attached technical specifications form for your compliance. bid should not exceed the total abc and should be treated as one 1 lot only. bid quotations should be inclusive of the 12% vat. please quote your lowest price on the items stated above, subject to the terms and conditions stated therein. your sealed quotation duly signed by your representative shall be addressed and submitted to the procurement planning and management division located at dotr office, unit 68, 6th floor, the columbia tower, ortigas avenue, brgy. wack-wack, mandaluyong city, not later than 3:30 p.m. of 11 july 2025 friday. take note that the submission of quotation by email is not allowed. copies of the following documents shall be submitted together with your quotation; to wit: 1. mayors permit or business permit; 2. philgeps registration number; 3. income/business tax return; and 4. omnibus sworn statement duly subscribed in accordance with the prescribed form under gppb resolution 16-2020 dated 16 september 2020 signed by the bidder or its duly authorized representative, accompanied by the duly notarized special power of attorney, board/partnership resolution, or secretary certificate, whichever is applicable. please see the attached revised omnibus sworn statement form. in lieu of the above-mentioned documents, philgeps certificate of registration and membership under the platinum category with updated class a eligibility requirements may be submitted.
