RFQ-X-Ray Walk Through
The Palacio del Gobernador Condominium Corporation PDGCC will undertake a Small Value Procurement for the Supply and installation of one 1 unit of X-ray Walk-through Metal Detector in accordance with Section 34 of the Implementing Rules and Regulations of Republic Act No. 12009. I. Project Description Supply and installation of one 1 unit of X-ray Walk-through Metal Detector II. Scope of Work/Technical Specification A. Scope of Work 1. Mobilization and demobilization; 2. Supply and delivery of one 1 unit of X-ray Walk-through Metal Detector; 3. Installation, testing, and commissioning; and 4. Submission of the final accomplishment report, including photographic documentation. B. Technical Specifications Particular Specifications Quantity X-ray Walk-through Metal Detector - 7-inch LCD display - 33 Detection Zones - Dimension: 2215mm x 938 mm x 535 mm - Multi-zone technology - Detection sensitivity: Up to 200 distinct sensitivity levels - Metal indicator light/body position indicator - Power: approximately 100VAC ~ 280VAC, 60Hz 1 unit C. Other Requirements: 1. The bidder must be a Filipino firm with at least three 3 years of continuous existence. **Bidders must submit a notarized certification to prove their years of existence with attached Registration certificate from Securities and Exchange Commission SEC or Department of Trade and Industry DTI for sole proprietorship. ** 2. The winning bidder shall conduct, at no additional cost to PDGCC, seminars and training for authorized PDGCC Building Administration personnel and technicians. 3. The winning bidder shall provide quarterly preventive maintenance services for a period of one 1 year upon the completion and acceptance of the project. III. Approved Budget for the Contract Three Hundred Forty-Three Thousand Pesos P343,000.00 Vat-Inclusive IV. Delivery Period Sixty 60 calendar days from receipt of the Purchase Order V. Warranty 1. The winning bidder shall provide a warranty against defects in materials and workmanship for a period of one 1 year. 2. The warranty period shall commence upon the issuance of the Certificate of Final Completion and Acceptance by the end-user. 3. All parts that fail or require replacement due to manufacturing defects or defects in workmanship during the warranty period shall be replaced by the winning bidder at no additional cost to PDGCC. VI. Project Location Palacio del Gobernador Cond. Corp., A. Soriano Ave. cor. General Luna St. Intramuros, Manila VII. Terms and Conditions 1. Payment shall be made upon submission of the following documentary requirements: a. Service Invoice / Billing Statement b. Certificate of completion and acceptance 2. Payment shall be processed in accordance with accounting and auditing procedures. No payment shall be released without the submission of the required BIR-registered receipt or other applicable tax documentation. VIII. Documentary Requirements Submission of quotation, together with the following documents, duly signed by you or your duly authorized representative, shall not be later than September 22, 2026; at 12:00 NN at the Administration Office, Basement, Palacio del Gobernador Cond. Submit at least three 3 Certificates of Completion from different clients for projects completed within the last three 3 years prior to the submission of the quotation. ? Quotation/Proposal Form Form A Note: • A valid PhilGEPS Platinum Certificate of Membership may substitute for both a Mayor’s Permit and a PhilGEPS Registration Number. • For recently expired Mayor’s Permits, please include the official receipt of renewal as proof of application within the required LGU period. • In case there is suspension of work due to declaration of special non-working day or fortuitous event on the scheduled deadline for submission and opening of Price Quotation, said deadline shall be moved to the next working day at same venue and time. IMPORTANT REMINDERS: 1. All entries must be legible. 2. Price validity shall be for a period of at least sixty 60 calendar days from submission and receipt of quotation. 3. Bids shall be evaluated by lot item 4. Bidder may include cost breakdown on the labor and materials as part of their Quotation. 5. Any interlineations, erasures, or overwriting shall be valid only if they are signed or initialed by the duly authorized representative/s. 6. Price quotation/s, to be denominated in Philippine peso, shall include all taxes, duties, and/or levies payable. 7. Quotations exceeding the Approved Budget for the Contract shall be rejected. 8. Place of Submission, Sealing and Marking: Quotations shall be sealed, and marked using the following format.
